The Bariatric Program Coordinator provides professional leadership in working with and educating the bariatric practice staff, hospital personnel, referring physicians, and patients. Additionally, this individual will provide oversight of ongoing Metabolic and Bariatric Surgery Accreditation and Quality Improvement Project (MBSAQIP) compliance, oversight of ongoing multidisciplinary team meetings for the bariatric surgery program and act as a liaison between the hospital and the Baylor St. Luke’s Weight Loss Program (BSLMC).

The ideal candidate will provide specialized care to the Bariatric patient population by effectively coordinating office activities. The goal will be to optimize patient outcomes through efficient practice management and clinical and educational expertise. The candidate will also contribute to the overall expansion of bariatric services and continued growth in the volume of patients.

Job Duties:

  • Manages Accreditation criteria to ensure the Baylor St. Lukes Medical Center (BSLMC) Weight Loss Program meets expectations set forth by governing agencies. Maintains and ensures compliance of all policies and procedures that guide and support the assigned services. These include, but are not limited to Joint Commission requirements, federal and state regulations, MBSAQIP standards, regulatory standards, etc.
  • Collaborates on an ongoing basis with Medical Director, clinic staff and physicians, promoting positive communication techniques and the ability to problem solve at every level.
  • Works with Medical Director to anticipate and identify the needs of the center and creatively implements solutions to problems. Integrates Evidence Based Practice into clinic improvements.
  • Provides orientation/education for center and hospital staff, students, and other hospitals and community organizations, as requested.
  • Administers nursing care to patients; includes assessment, planning, implementing, and evaluating the patient’s care plan in coordination with the physician. May or may not be applicable to each position.
  • Provides patient/family or caregiver education and counseling in recognition and solution of physical, emotional, spiritual, and environmental health problems.
  • Works independently with limited direct supervision using the Nurse Practice Act, Hospital or Practice policies, nursing standards, and professional judgment.

Minimum Qualifications:
Bachelor’s degree.
Four years of relevant experience.

Preferred Qualifications:
Five years of experience as a licensed healthcare professional with at least three years in bariatrics.
License as a Registered Nurse or Registered Dietician.

Qualification Requirements:
-MINIMUM EDUCATION: Completion of a minimum of a Baccalaureate degree granted by a U.S. regionally accredited college/university or foreign equivalent

Registered Dietician will integrate into multidisciplinary bariatric and metabolic diseases clinic providing patient assessment and education. Dr. Rogalsky, our center’s bariatric surgeon, performs approximately 175 bariatric operations yearly in addition to having a busy outpatient and inpatient general surgery practice. Position Summary:

This individual is responsible for the peri-operative psychosocial assessment which is designed to screen and identify potential psychosocial risk factors which may impact successful completion of the program. The Clinical Social Worker develops mutually defined goals with the patient in collaboration with the interdisciplinary team to address these potential areas of concern in support of the patient’s weight loss journey. The qualified candidate will have experience in a medical setting as well as expertise in evidence-based practices relevant to bariatric social work and cultural competence in care delivery.

Responsibilities:

1. Initial and follow-up Psychosocial Assessments to evaluate a patient’s preparedness for medical weight loss interventions

2. Recognizes and diagnoses relevant behavioral health conditions using the DSM-V-TR

3. Collaborates with the providers and other members of the multidisciplinary team to establish measurable healthcare goals and prioritization of the patient’s needs as well as identifying any barriers to care

4. Responds to patients in crisis when needed

5. Knowledgeable of the mandating reporting process for adults, children and disabled persons.

6. Uses evidence-based practice with patients and/or family members for short term therapy when necessary

7. Facilitates or co-facilitates groups based on the Weight and Wellness practice needs

Support the nutrition component of the Woodhull Bariatric Program.  

Summary of Duties and Responsibilities:

Under the guidance and supervision of the Associate Director and Chief of Bariatric Services:
• Perform full nutrition assessments for patients seeking bariatric surgery.  Assessments include, but are not limited to gathering nutrition history, medical history, laboratory data, medications, meal patterns and all other relevant data.
• Maintains and ensures compliance of all policies and procedures that guide and support the assigned services. These include, but are not limited to Joint Commission requirements, federal and state regulations, ASMBS-AQIP standards, regulatory standards, etc. 
• Works with the Chief of Bariatric Service to coordinate and establish ASMBS standards, policies and procedures to ensure program is in path for Center of Excellence certification.
• Determine nutrition problems food/nutrient drug interactions, therapeutic interventions, create individualized treatment plan and develop related patient goals.
• Considers cultural and religious food preferences, language barriers, physical disabilities and age group when performing assessment or providing management. 
• Counsel, monitor and evaluate progress each patient is making toward established goals and expected outcomes pre- and post-op.
• Accurately and comprehensively document results of nutritional assessments and patient counseling in the EPIC medical record in accordance with Department Policies, Joint Commission and CMS guidelines.
• Facilitate group nutrition education classes for patients using approved materials and/or curriculum.
• Works with the Department of Surgery’s Service Line Administrator to ensure that Center of Excellence standards and Joint Commission standards are met with regard to all regulatory requirements.
• Works with the Bariatric team to anticipate and identify the needs of the center and creatively implements solutions to problems. Integrates Evidence Based Practice into clinic improvements.
• Coordinate with the Associate Director of Bariatrics to manage patient meetings, appointments, pre-screenings, surgical dates and all other tasks needed to move the surgical process forward.
• Provides orientation/education for center and hospital staff, students, and other hospitals and community organizations, as requested.
• Demonstrates effective teaching methods for multi-level individuals and/or groups for delivering effective personalized, patient-centered, family-focused outcomes and professional development of colleagues. 
• Support Departmental interdisciplinary performance improvement goals for Bariatric Surgery Clinic
• Complete necessary paperwork maintains records to support insurance billing/reimbursement process as needed.
• Gather population specific nutrition team meetings as appropriate and as determined by supervisors. 
• Develop populations specific nutrition education materials as needed, including working with Public Affairs to update and provide motivational materials on website and social media platforms.
• Attend mandatory in-service training, assigned committee meetings, in-services and HR related trainings as required. 
•  Maintains patient confidentiality and discards Protected Health Information (PHI) as per hospital policy. 
• Performs related tasks as assigned by the Chief of Surgery, Surgery Service Line Administrator or Designee.

Qualification Requirements:

For appointment to Assignment Level I
1. Baccalaureate degree from an accredited college or university, with a major in Dietetics or Foods and Nutrition in a program approved by the American Dietetic Association (ADA); or,
2. Registration or eligibility for registration by the Commission on Dietetic Registration (CDR) as a Registered Dietitian or New York State Certification as a Dietitian-Nutritionist.

For appointment to Assignment Level II
1. Registration or eligibility for registration by the CDR as a Registered Dietitian or New York State Certification as a Dietitian-Nutritionist.

For appointment to Assignment Levels III and IV
