Identifies relevant psychosocial factors that may present challenges and impede patient’s progress. Develop treatment plans that involve teaching self-monitoring behaviors, changing old beliefs, building new coping skills, and making changes to home and work environments to support health goals. Help patients address obstacles to weight loss, identify positive ways to change unhealthy eating habits, and develop new skills and ways of thinking. Leads programs and classes related but not limited to emotional eating disorders, binge eating, coping with stress eating disorders, and develop individualized and group models treatments models that address stress and weight loss treatment options. Our multidisciplinary team will provide medical, nutritional, psychological, physical training, and surgical treatment. Duties and responsibilities:

· Assesses and monitors the continuity of care and expedites and coordinates the delivery of services to facilitate a patient’s progress throughout the continuum of care, from point of first interest or referral, through the qualification period, the surgery process, post-op care, and post discharge to achieve desired quality outcomes. Coordinates insurance approval for medical and surgical care.
· Assists in administrative functions and the development and implementation of patient care protocols.
· Assists in the identification and reporting of quality issues, and in the development of a performance plan for the Bariatric Surgery Program to assess and improve the quality and efficiency of the program including but not limited to quality assessment, utilization review, risk management, and infection control. Identifies appropriate indicators to monitor and track and in conjunction with marketing, tracks patient referral outcomes.
· Maintains MBSAQIP data for COE certification and re-certification.
· Works in conjunction with and assists leadership and clinical nurse specialists in developing curriculum and providing in-service education for Medical Staff and employees regarding the special needs of Bariatric patients. (Sensitivity training, lift & transfer techniques, complications of Bariatric surgery, post-op care psychosocial needs etc.).
· Coordinates and participates in monthly patient education classes.
· Coordinates and participates in monthly Support Groups. Collaborates with the patient support group facilitator and facilitates meetings as necessary in the absence of a facilitator.

Additional Requirements:

· Current Washington State Registered Nurse license.
· BSN preferred.
· Three to five years of clinical experience, management experience, or quality management, case management or discharge planning experience.
· Experience in program development and insurance approvals.
· Experience in obtaining and/or recertifying Bariatric Center of Excellence status a major asset.
· Current American Heart Association BLS/CPR for Health Care Providers certification
· Two years of recent clinical experience as an RN working in a clinic/outpatient setting preferred.
